Follistatin is a vital single-chain gonadal protein that plays a key role in regulating the release of follicle-stimulating hormone (FSH). This protein is encoded by the FST gene, which produces two distinct isoforms?FST317 and FST344?comprising 317 and 344 amino acids, respectively, due to the alternative splicing of precursor mRNA. Follistatin binds directly to activin, acting as an antagonist that effectively inhibits the production and secretion of FSH from the pituitary gland.

Current research is exploring follistatin's significant role in muscle growth regulation. Studies in mice have shown that it acts as an antagonist to myostatin (GDF-8), a member of the TGF superfamily known for limiting excessive muscle development. Pioneering research by Lee & McPherron demonstrated that either genetically eliminating GDF-8 or increasing follistatin levels resulted in considerable muscle mass gains. Additionally, a 2009 study involving macaque monkeys revealed that gene therapy aimed at regulating follistatin also enhanced muscle growth and strength.
